2022 Photo/Video Release Form
AUTHORIZATION TO USE PHOTOGRAPHS AND/OR AUDIO-VISUAL

I, ______________________, hereby authorize Marc Faubel (photographer-Marc Faubel), ThatHeadshotGuy Photography, or Marc Faubel Photography to use, reproduce, and/or publish photographs and/or video that may pertain to me including my image, likeness and/or voice without compensation. I understand that this material may be used in various publications, public affairs releases, recruitment materials, broadcast public service advertising (PSAs) or for other related endeavors. This material may also appear on the company's or project sponsor's Internet Web Page. This authorization is continuous and may only be withdrawn by my specific rescission of this authorization in writing. Consequently, ThatHeadshotGuy or the project sponsor may publish materials, use my name, photograph, and/or make reference to me in any manner that the company or project sponsor deems appropriate in order to promote/publicize service opportunities. ThatHeadshotGuy will NOT sell my images to any third party or make a profit directly from my likeness. I understand that I may share but not edit/crop images created by ThatHeadshotGuy/Marc Faubel and use on social media provided I link back to ThatHeadshotGuy Facebook page as well as “tagging” Marc Faubel directly but may not print any image in addition to the agreed upon prints or digitals I am already receiving without purchasing the copyright to said image from Echo Reality Photography first. I also acknowledge the print release price per purchase is $50.00*. I acknowledge that if I desire printed copies of any image made by Echo Reality Photography and choose to opt out of the print release that I may contact them and order prints directly through ERFOTOS.com.
I have agreed to accept digital delivery of my image(s) taken by Marc Faubel/ThatHeadshotGuy. Personal non-profit use is authorized by Marc Faubel of ThatHeadshotGuy and photos used for project specific ventures need not be tagged on social media but photo credit should be given where possible. I may use my photos to obtain work such as auditions but will NOT sell images taken by Marc Faubel/ThatHeadshotGuy without written permission. I do not grant creative permission to alter/crop the photograph(s).
I do not grant permission to resell or use the photographs in any manner including a manner that would exploit or cause malicious representation toward me or my company and associates. My digital image may be used across any social media platform that I so choose but when used on Facebook, Twitter or Instagram, I will link back to the following links:
